    Abstract [en]

    About eight percent of all male Swedish eighteen-year-olds are exempted from military service on a psychiatric basis annually. From the perspective of the labelling theory of mental illness, the aim of this paper was to study some consequences on other people's perception of individuals who have been exempted on this basis. An experiment was performed with two different groups of subjects; 81 conscripts and 61 students of personnel administration. The subjects were to listen to a tape-recorded description of a car accident and to evaluate its causes. A brief description of the life history of the driver was given which included a “label”. It was foud that with a psychiatric exemption label the accident was attribuited significantly more to factors internal to the driver than to external factors, as compared to a general problems-living label.

    Abstract [en]

    The purposes of this study were twofold. The first was to investigate if relaxation training conducted by nurses as an integral part of their caring activities affected breast cancer patients' experiences of radiotherapy sessions as well as their general wellbeing. Second was to evaluate a pedagogical model for large scale application of relaxation training as an integral part of caring. The sample consisted of 64 consecutive outpatients with breast cancer at a Swedish oncology clinic, receiving their first radiotherapy treatment course following surgery. Every second patient was assigned to an experimental group (n= 32, mean age: 59.5 years) and every other second patient was assigned to a control group (n= 32, mean age: 60.0 years). The programme resulted in the following general effects: fewer perceived daily hassles and a more cheeful overall mood state. The following treatment situation specific effects were noted: programme participants appraised the treatment sessions as successively more benign-positive and less threatening. They also reported more perceived muscular tensions during treatment sessions. Most goals concerning the routinization of the pedagogical model were reached.

    Abstract [en]

    The aim of the study was to develop a theoretical understanding of conditions and mechanisms affecting experiences of the quality of emotional debriefings, using a grounded theory approach. Twenty interviews were conducted with experienced male debriefing leaders and participants in the Stockholm fire brigade and the Swedish armored United Nations battalions serving in Bosnia. Data were analyzed according to the constant comparative method. A model was formulated according to which the quality of debriefings is formed by the dynamic interplay between group and debriefing leader characteristics. The key group and debriefing leader quality is security. Furthermore, individual group member resourcefulness and vulnerability, together with the degree of knowledge and support from the management of the organization, also affect experiences of the quality of debriefings. The model is discussed in relation to existing thoughts on potentially favorable mechanisms.

    Abstract [en]

    The aim was to develop a theoretical understanding of the decision-making process leading to appendectomy. A qualitative interview study was performed in the grounded theory tradition using the constant comparative method to analyze data. The study setting was one county hospital and two local hospitals in Sweden, where 11 surgeons and 15 surgical nurses were interviewed. A model was developed which suggests that surgeons’ decision making regarding appendectomy is formed by the interplay between their medical assessment of the patient's condition and a set of contextual characteristics. The latter consist of three interacting factors: (1) organizational conditions, (2) the professional actors’ individual characteristics and interaction, and (3) the personal characteristics of the patient and his or her family or relatives. In case the outcome of medical assessment is ambiguous, the risk evaluation and final decision will be influenced by an interaction of the contextual characteristics. It was concluded that, compared to existing, rational models of decision making, the model presented identified potentially important contextual characteristics and an outline on when they come into play.

    The aim of the study was to explore the relationships between early attachment between mother and daughter, the adult daughter's general anxiety level, and characteristics of the helping interaction between the adult daughter and her elderly mother on the one hand and the quality of this help on the other. One sample consisted of 46 Swedish women (mean age: 55 years) who acted as informal caregivers for their elderly mothers. A second sample consisted of 106 Swedish female nursing students (mean age: 27 years). The participants rated the quality of the practical, physical and psychosocial help they gave their elderly mothers (imagined helping activities in a predefined situation among the nursing students). Subjects also rated their mother's degree of caring and overprotection in childhood, their own trait anxiety, and various characteristics of their helping interactions. A high degree of motherly caring and a low degree of motherly overprotection in childhood covaried with a lower level of trait anxiety in the adult daughters. These conditions, in turn, covaried with mutual friendliness during the helping interactions. All these conditions covaried with high-quality psychosocial support. The quality of practical help and physical caring covaried with friendliness during the helping interactions only.

    Abstract [en]

    The aim was to study the relationship between elderly home care users', and their caregivers', perceptions of the quality of care. The sample consisted of 151 matched elderly home care user-caregiver pairs in a Swedish municipality. The elderly home care users were interviewed and their caregivers filled in questionnaires using an established, theory-based instrument. Results showed that the elderly home care users evaluated most care components more favourably than their caregivers. On ratings of the various care components' subjective importance to the caretaker, the caregivers consistently scored higher than the elderly home care users. Within the subset of elderly home care users who received help at least twice a day, there were greater similarities between caregivers and caretakers. The results are related to comparable research and discussed in terms of caregivers' needs to legitimize their professional identity and actions.

    In 1994, the questionnaire 'Quality of Care from the Patient's Perspective' (QPP) was developed using a conventional factor analytical approach (Wilde et al. 1994). The items and conceptual framework of this questionnaire were derived from a theoretical model, which, in turn, was developed from qualitative patient interviews, using a grounded theory method of analysis (Wilde et al. 1993). The aims of this study were to develop further the dimensionality of the QPP using structural equation modelling combined with advances in factor analysis modelling, and to refine the instrument. This comparatively new methodology extracts more information out of questionnaire data and is considered to be superior to traditional scaling methods. The sample consisted of 611 somatic inpatients from four departments at a Swedish hospital. Data collected with the QPP were explored with the new tool tracing the dimensions postulated in the theoretical model underlying the QPP. A nested factor model was developed fulfilling statistical criteria for adjustment between model and data. The model consists of a general factor and 16 subordinate factors. Most of the original QPP scales were reproduced with the new statistical technique. However, some content changes were made which appear to bring the QPP scales closer to patients' meaning representations of the area. Some changes were also made to the response format, changes which appear to strengthen the psychometric properties of the instrument. The refined QPP will hopefully contribute to a more differentiated picture of quality of care when applied in the field. All scales, items, and response formats of the revised QPP are presented in an appendix.

    Abstract [en]

    Rational and aim: Knowledge of which aspects of care are more important to patients is crucial for health providers. The aim of the work presented here was to explore changes over time in patients’ preferences in hospital and in out-patient settings.

    Method: Cross-sectional data were collected among patients at a number of Swedish hospital wards and out-patient clinics in 2003, 2004, 2005 and 2008 (N = 46,525). Data for men and women were analysed separately. The main outcome measure consisted of replies to the questionnaire “Quality from the Patient’s Perspective” (QPP) (numerous references).

    Results: (1) Subjective importance (preference) ratings increased throughout the time period studied, (2) in-patients gave higher ratings than out-patients and (3) women gave higher ratings than men, particularly in out-patient settings.

    Conclusion: The increase over time in ratings of the importance ascribed to different aspects of care may reflect a general trend towards strengthened consciousness among citizens as consumers of care and is an important index for improving the person-centeredness of clinical services.

    Existing methods for handling patients’ opinions as a basis for quality improvement measures tend to be too sophisticated for practical nursing and/or weak psychometrically. An Index of Measures was developed by combining patients’ perceptions of actual care conditions with the subjective importance they ascribed to these conditions. Data from Swedish somatic inpatients (n = 4002) indicated that this index gave added value compared to patients’ ratings of actual care conditions only. Interpretation of the Index of Measures focused on the proportion of patients who receive the value ‘deficiency.’ This value is obtained if a given patient has a subjective importance rating on a given item which is higher than the mean subjective importance score for the whole patient group on that item, as well as a perceived reality rating on the item in question which is lower than the mean perceived reality rating for the whole group on that item. Guidelines are suggested on when improvement actions are necessary and when they are not necessary.

    Abstract [en]

    Purpose

    The purpose of this study was to develop an easy-to-use, theoretically well-founded and psychometrically sound assessment tool of the concept co-workership.

    Design/methodology/approach

    Firstly, inductively generated examples of favourable and unfavourable co-worker behaviours were collected, clustered and then expressed as frequency-based Likert-type scale items. Data were obtained from 825 Swedish white collar workers and military personnel. A factor analysis (principal axis factoring with oblique rotation) resulted in a seven-factor solution built up by 30 items and forming the instrument Co-Worker Questionnaire (Co-Worker Q).

    Findings

    The factors have a strong resemblance to the content of dominating models of leadership, followership, organizational citizenship behaviour and leader–member exchange. The factor scales had relatively high reliability (high Cronbach’s alpha coefficients and low standard errors of measurement). Regarding discriminability, women scored more favourably on five of the factors, men on one factor and age differences were noted on two factors.

    Research limitations/implications

    Shortcomings include the lack of data on possibly related aspects including person factors, such as personality and socio-economic status, and contextual factors such as more detailed data on the type of work environment and organizational culture.

    Practical implications

    The instrument has an easy-to-use format and can be used in organization development interventions with a co-workership focus and in individualized coaching or mentoring programmes.

    Originality/value

    The co-workership concept has so far mainly been used in the Nordic countries. The development of the Co-Worker Q opens up for broader applications.

I dagens globaliserade samhälle, med avancerad teknologi och högutbildade kompetenta medarbetare, ser det annorlunda ut. Enskilda medarbetare och grupper av medarbetare förväntas aktivt ansvara för att tänka ut och planera vad som ska göras, sedan göra detta självständigt och därefter värdera resultatet i en lärande spiral. Ett utvecklande medarbetarskap med stor självständighet och stort ansvarstagande är därmed en viktig faktor för organisatorisk framgång.

    I denna bok summeras teori om medarbetarskap och därefter presenteras detaljerade förslag på hur man kan arbeta praktiskt i en organisation med att utveckla medarbetarskapet så att det samspelar med ett modernt, utvecklande ledarskap.

    Abstract [en]

    In this study, well-known psychosomatic (psychoneuroimmunological) connections have been used for therapeutic purposes. Fifteen patients with rheumatoid arthritis (RA) were treated with analytical insight therapy for 60-90 min weekly during 10 months to 4.5 years (median 3.3 years). Six patients showed great and largely continuous improvement, both psychic and physical, throughout the period of psychotherapy and often for a long time thereafter. In another five cases, improvements in psychic state and the joint disease took place over a period of 1-2 years. Later, when the emotional channels to childhood traumata were opened, these five patients relapsed and the course of their disease became more variable. In four cases, the connections between psychic state and the course of the joint disease were more varied throughout the period of psychotherapy. In the studied group the correlation between psychic state and course of rheumatic disease was highly significant. The probability of the outcome of the study being due to chance alone is considered very small, especially taking into account the fact that RA is a chronic progressive disease.

    STUDY OBJECTIVE--To explore the link between serum cholesterol and suicide by investigating the relation between serum lipids and depressive symptoms. DESIGN--This was a cross sectional study of the relation between serum cholesterol, low density lipoprotein (LDL) cholesterol, high density lipoprotein (HDL) cholesterol, and triglycerides on the one hand and depressive symptoms as expressed in a questionnaire on the other. SETTING--An organisational development programme in industry with assistance from occupational health care. PARTICIPANTS--A total of 644 male and 261 female employees (mainly white-collar workers) participated. MAIN RESULTS--Total cholesterol and LDL cholesterol values were lower in those men who, sometimes, often, or very often, had experienced low mood or glumness during the past month compared with those who had not. Serum triglyceride concentrations did not differ between the groups. In women, however, the serum triglyceride value, but not the total cholesterol or LDL cholesterol, was lower in those who reported low mood, depression, or anxiety during the past six months. CONCLUSIONS--Decreasing appetite as a consequence of depression in men would probably lead to both decreasing cholesterol and triglyceride concentrations. Thus, these data indicate the presence of some other explanation for the relation between the level of LDL cholesterol and depressive symptoms in men.

    Abstract [en]

    This study reviews empirical evidence for Moghaddam’s model “Staircase to Terrorism,” which portrays terrorism as a process of six consecutive steps culminating in terrorism. An extensive literature search, where 2,564 publications on terrorism were screened, resulted in 38 articles which were subject to further analysis. The results showed that while most of the theories and processes linked to Moghaddam’s model are supported by empirical evidence, the proposed transitions between the different steps are not. These results may question the validity of a linear stepwise model and may suggest that a combination of mechanisms/factors could combine in different ways to produce terrorism.
